Products Eligible for Sampling

Through our free sampling program provided to ALL of our patients, you can receive samples and patient education materials. These product samples and materials are provided to you at no charge to test patient tolerance. Eligibility conditioned upon the intended individual and while supplies last. This sampling program is subject to change or cancellation at any time.

Please read carefully the following terms and conditions applicable to all Sampling Nutrition products provided to you:

• Certain products are classified as medical foods. Medical foods are intended for the dietary management of certain diseases or conditions and should only be consumed as directed by a healthcare professional. Medical foods are not intended to diagnose, treat, cure or prevent disease.
• Samples are for patient use only
• Samples are provided solely to aid in determining whether the product is clinically appropriate (or deemed medically necessary) and/or whether it will be accepted or tolerated by the patient.
• Coupons will be provided to support on-going therapeutic support.
• SAMPLES ARE NOT FOR RESALE.
